1. Customers purchasing a selected Sharp Aquos LED HD Ready 1080p LCD at participating stores or online beginning 21st Ocober 2009 (the "promotional period") may claim £100 cashback, (in the form of a cheque). All purchases and mailing addresses must be within the UK and Republic of Ireland.

3. 6. To claim your cashback you must send a fully completed claim form and your invoice or receipt as proof of purchase. You must complete the claim form by inserting your name, address, email address, contact telephone number, model number of the LC D purchased, the date and place of purchase. Proof of purchase submitted must detail the qualifying product purchased, the retailers name, address and telephone number, the purchase and price paid. Order confirmations and delivery notes constitute proof of purchase.
